A Case of Chronic Congestive Splenomegaly



Abstract
Chronic congestive splenomegaly is characterized by enlargement of the spleen, progressive anemia, leukopenia, often thrombocytopenia, gastrointestinal hemorrhage caused by portal hypertension, and in later stages, cirrhosis of the liver and ascites.
We experienced a case of chronic congestive splenomegaly in which definite cause of portal hypertension couldn¢¥t be found out in 9 year-old male patient.
The chief complaint on admission was abdominal mass, hematemesis, and nasal bleeding. On physical examination, the spleen was palpated 7cm under the left subcostal margin with mild tenderness.
The diagnosis was made by clinical, laboratory and radiologic findings.
A mesocaval shunt was performed and thereafter portal pressure was reduced from 40 cm H2O to 24 cmH2O and also, the spleen was dramatically reduced to normal size. A brief review of related literatures was presented.
